This is definitely a Luo case. In fact it is one of the worst examples of Luo pathology I have seen recently. If you cannot bleed these vessels because it is not in your scope of practice, then you must focus heavily on the harmonization and circulation of blood. This might also be handled in the absence of bleeding with the 8EV.
I also would strongly address circulation in the chest.
