    Idiopathic hypogonadotropic hypogonadism (IHH)

    I would love your thoughts on how to approach supporting this 39 yo male. Dx with Idiopathic hypogonadotropic hypogonadism (IHH) / secondary when he was 29yo and had 3 sperm samples frozen.

    This past year went through 1 round of IVF with his partner and the sperm has DNA fragmentation and low motility.

    Naturally testosterone is low and over the years he has taken exogenous testosterone. The couple wants to take a break from IVF and work on overall boosting fertility health for both of them. Going to suggest Herbal formulas to support him over (at least a 3 month course) with lifestyle modifications (and hopefully acupuncture)!

    Q: Has anyone supported male factory with this presentation clinically? How long does it take for a man to increase testosterone after taking exogenous ( reading as long as 12 months). I understand this his increase may be on the lower end of what’s ideal d/t his diagnosis.

    Q: Considering to ask him for new semen analysis after a course of treatment. Do you think 3 months too short of a time frame with his dx?

    Q: What do RE’s typically do in these instances for male factor? Reading FSH and GnRH agonists meds to help. Would TESA be a good option as fragmentation is less when extracted from testes?
