Thanks for your thoughtful response, Sarah! And yes, definitely confusing. She was told that she had a circumvallate placenta and was also told it was normal. At least, that is what she reported to me. So, I was eager to talk to the midwives, and unfortunately, never heard back from them. However, when I followed up with her to let her know I was not able to get in touch with the midwives, she said she had just come from an appointment with them and baby flipped! So, she canceled her follow-up appointment with me.

I had those 2 scenarios as possibilities in my head as well, either things were less severe than they sounded, or the midwife was under the impression that acupuncture and moxa would have been safe. Either way, the take home message on this case for me has been everything we can do, even when it is contraindicated to give a breech treatment. Thanks for the support!

Thanks for both of your inputs! I put a call in to her midwife group to find out more about the rationale for the moxa treatment recommendation. However, the more I am learning about this placenta abnormality, the more I understand that this is certainly contraindicated.

Which brings me to another question: would you advise she stay away from other avenues to help turn baby, such as spinning baby or the Webster technique? I did reach out to a chiropractor and asked if this patient could be safely treated with the Webster technique, and she said it wouldn’t be contraindicated because they are just making space in the pelvis, not trying to turn baby.

This patient told me she was told she still has a normal placenta and to do whatever she can on her own to help get baby to turn. There has also been no discussion on the possibility of a C section with her. I explained to her why it did not seem like she would be a good fit for the moxa protocol, but shared everything that we could safely do. So, I will be curious to hear the midwives perspective on this one!

  • Thank you both so much for these thoughts and resources. I agree with you, Maria, on her deeply deficient presentation, those herb recommendations sound like great places to start. And Sarah, good to know about the blue cohosh! It makes sense that dealing with my patient’s case is a different scenario than it being used to induce labor. But I really appreciate learning the whole story of the herb, and when to make sure you’re using it with caution. I think, in general, when most people hear the words “herbal tincture,” they assume all are gentle and safe, so thanks for shedding this light! Also, she did not mention anything about a backup formula in case there was too much bleeding. Such an important thing to keep in mind, and I love your idea on the press tacks on SP1 and making sure they understand it is not a substitute for an ER visit. Hopefully, I will be checking in with the patient today and will keep you posted on updates.

I most likely would not be able to make it, but would love to benefit from watching a recording if that’s possible!

    Hi everyone!

First of all, I was bummed that I had to miss the live class today, looking forward to meeting you all at the next meeting! It sounds like I am also one of the newer practitioners here. I graduated from Bastyr University in 2019, then worked at a women’s health acupuncture clinic up until recently starting my own practice here in Bellingham, WA. My focus has always been in women’s health, and now that I am starting my own practice, I am very grateful to be a part of this mentorship program. I am also hoping to sit for the ABORM test in 2023. Excited to learn from you all and meet you soon!

Hello! Thanks for following up on this. The patient was in the middle of moving states (!) when all of this was happening, so I only saw her twice. However, I followed up with her, and she told me the day after her second acupuncture visit, her body went into labor on her own, but ended in a trip to the ER. I did not ask for specifics, as she mentioned she was exhausted and just wanted to rest. She said they gave her an ultrasound once everything calmed down, which showed everything had passed that needed to. She did mention that even though it ended in the ER, she did not go with a fearful heart and credited the acupuncture for that.
